How many DS-160 forms does the entire family need?

There is no single form for the whole family. In the L-1 visa process and the L-2 (dependents) process, every person applying for a visa must complete their own DS-160 form, individually.

This applies to every family member included in the application: the L-1 principal, the spouse, and each child, including infants and young children. Each applicant generates their own form with their own confirmation number, because the DS-160 is tied to a single applicant and serves as the basis for the consular interview and the individual review of each person.

In practice, count how many family members will be applying for a visa: that is the number of DS-160 forms to complete. Parents typically fill out and sign the forms for minor children, but it is still one DS-160 per dependent.

Complete each form carefully and review the official guidance from the Department of State and the consulate before scheduling the interview, as instructions and details may be updated.
