Do L-2 children need a medical exam?

For dependents on an L-2 visa, including children, a medical exam is not typically required as part of the visa application. The L-2 accompanies the L-1 principal and follows the standard nonimmigrant visa process.

A medical examination is usually mandatory at a different stage: the adjustment of status to permanent residence (green card). At that point, the exam is used to verify public health requirements under immigration rules, which are not part of the L-2 phase.
