I am a research physician in Brazil but have not taken the USMLE. Does EB-1B require medical licensure in the U.S.?

The EB-1B is the category for outstanding researchers and professors, and its focus is academic contribution, not the practice of clinical medicine. For a physician who works as a researcher, the core of the case rests on their achievements in research and teaching.

Therefore, when the intention is to pursue research or academia, EB-1B does not require the USMLE or a license to practice medicine in the United States. What must be demonstrated is recognition of your work:
